getchell book 1 & 2 or the early hering books (forgot their titles).
first, read through the etude (or just a line at a time) humming/singing while fingering. then play. by book's end you'll be able to play these by sight, then move to something harder.

I know I had to sight read for 2 honor bands. It wasn't too hard. Just relax and remember s.t.a.r.s. S- sharps and flats, T- time sig.,A- articulations, R- rhythms, S- special markings( accidentals and garce notes). It really helps when you only have a few mintues to look at a piece. I was in 4rd of 21 my 7th grade year and 3rd of 24 my 8th grade year. Hope this helps. Also study rhythms and time sigs. that aren't in 4/4 time.
